Maplestory Registering Translation help please.

The only parts I understand is the top blank is Login name and I finished that part.

Second 2 I guess are the Password ones. Can somone please translate the req. for passwords, in case it has to be aplha-numeric.

I have no complete idea on the last 2, can I have a translation and instructions on what to do? All I can guess is click the gray boxes probly needs both my adult and Kid KSSN's. (I had a Korean Nexon account for a while, just never did a think with it)
